I remember seeing something about data loss issues being discovered in 1.4. Is that still an issue?

There are no data loss issues that I'm aware of in 1.4.0.
You might be thinking of the data loss issues we detected doing comparison testing with HBase. Those losses were in HBase and were reported to their team (HBASE-5754).
There is a failure that can occur if you have a large instance, but this just prevents tablet assignment: there is no data is loss, and a work-around has been documented (ACCUMULO-542).
